Centenarian Celebration

Do you know someone living in Chesterfield County who will be 100 years of age or older this year? We want to honor them! The Chesterfield County Department of Citizen Information & Resources, Office of Aging & Disability Services, is seeking centenarians to recognize and honor.

Recognize a Centenarian

Tell us about a centenarian’s life well lived by sharing their history and distinctive experiences and achievements. To receive the Centenarian Certificate and be recognized, a social history form must be completed. 

Download and complete the applications. This submission is in two parts - please send them separately:

  1. Complete the Centenarian Recognition Request by August 7, 2020 
  2. Next, complete the Centenarian Social History by August 17, 2020

Special Recognition at the Board of Supervisors Meeting
