12. PER OUR PHONE CONVERSATION, PLEASE RE-EVALUATE IF A
12" PARALLEL WATER LINE IS NECESSARY. THE CONFIGURATION O
LOOPED 8" LINES ON THE SITE WITH CONNECTIONS ON BOTH ROUTE
10 AND ROUTE 1 ALLOWS OVER 3,000 GPM FIRE FLOW TO THE SITE
IF POSSIBLE, THE WATER LINE ON THE REAR OF THE SITE SHOUL
BE RELOCATED AND TIED BACK INTO THE 8" LINE THAT LOOPS
AROUND THE FRONT OF THE BUILDING USING A TEE, TWO SLEEVES
AND TWO VALVES.
13. THE PROPOSED LOCATIONS OF THE FIRE HYDRANTS AT STA
10+24 AND 13+96 ARE NOT ACCEPTABLE AS THEY ARE DEPICTED TO
BE SET UP INTO A SIDE SLOPE.
14. THE UTILITIES DEPARTMENT HAS HAD DISCUSSIONS REGARDIN
THE CO-LOCATION OF OTHER UTILITIES WITHIN THE WATER
EASEMENT; HOWEVER, WE DO NOT SEE THIS SHOWN ON THE PLANS.
PLEASE SHOW THE PROPOSED LOCATION OF THE GAS LINES AND
ELECTRIC AS DISCUSSED. NOTE THAT A LICENSE AGREEMENT WILL
BE REQUIRED THROUGH OUR RIGHT-OF-WAY DEPARTMENT FOR THESE
CO-LOCATIONS.
15. LABEL THE EXISTING ADJACENT PROPERTY OWNERS, JEFFERSO

18. A 1-1/2" METER IS ADEQUATE TO SERVE ALL THREE TENANTS
BASED ON THE FIXTURE COUNT PROVIDED. THE OWNER MAY CONSIDE
CONSOLIDATING TO A SINGLE ACCOUNT AND INSTALLING PRIVATE
METERS ON TENANTS 1 AND 3 IN ORDER TO SPLIT THE UTILITY

54. PERMISSION MUST BE OBTAINED FROM THE ADJACENT PROPERTY
OWNER(S) ALLOWING FOR GRADING ON THE PROPERTY AS
PROPOSED PRIOR TO ISSUANCE OF A LAND DISTURBANCE
PERMIT.
55. PRIOR TO ISSUANCE OF A LAND DISTURBANCE PERMIT, THE
NAME, "WALK-IN" ADDRESS, AND TELEPHONE NUMBER OF
A LOCAL REGISTERED AGENT REPRESENTING THE OWNER FOR
SERVICE OF PROCESS MUST BE PROVIDED ON THE "APPOINTMEN
OF AGENT FOR SERVICE OF PROCESS" FORM FOUND ON THE
CHESTERFIELD COUNTY ENVIRONMENTAL ENGINEERING WEBPAGE.

56. THE POLLUTION PREVENTION PLAN MUST BE PROVIDED TO THE
ENVIRONMENTAL ENGINEERING INSPECTOR PRIOR TO ISSUANCE
OF A LAND DISTURBANCE PERMIT. THE CHESTERFIELD COUNTY
POLLUTION PREVENTION PLAN SHEET MAY BE PROVIDED IN THE
CONSTRUCTION PLANS (THIS IS AVAILABLE ON THE
ENVIRONMENTAL ENGINEERING DOCUMENTS AND FORMS WEBPAGE)
57. AS-BUILT BMP INFORMATION WILL BE REQUIRED FOR EACH
BMP FACILITY AS PART OF ITS FUTURE CERTIFICATION.
58. PRIOR TO ISSUANCE OF A LAND DISTURBANCE PERMIT, A
SURETY FOR THE CONSTRUCTION OF THE PERMANENT BMP MUST
BE POSTED WITH ENVIRONMENTAL ENGINEERING. (COUNTY CODE
PART II, CHAPTER 8, SEC. 8-16)
59. THE BMP MUST BE CERTIFIED PRIOR TO RELEASE OF THE BMP
PERFORMANCE BOND.
60. PRIOR TO ISSUANCE OF A LAND DISTURBANCE PERMIT, A COPY
OF THE AUTOCAD FILE MUST BE SUBMITTED TO CAROLE
CLIBORNE OF ENVIRONMENTAL ENGINEERING. THE AUTOCAD
FILE MUST BE SAVED AS A .DWG OR .DXF IN AUTOCAD VERSIO
2009 OR EARLIER AND REFERENCED DIRECTLY TO THE VIRGINI
STATE PLANE COORDINATE SYSTEM, SOUTH ZONE IN THE NAD83
DATUM. THE AUTOCAD FILE MAY BE SUBMITTED ON A CD, DVD
OR VIA EMAIL AND MUST BE CLEARLY LABELED WITH THE SITE
PLAN NAME, SITE PLAN NUMBER, ENGINEERING FIRM AND CAD
FILE PREPARER'S NAME AND PHONE NUMBER. THE FILE MUST
CONTAIN THE FOLLOWING, EACH CLEARLY LABELED IN SEPARAT
LAYERS:
A. PROPOSED BUILDING FOOTPRINT;
B. PARKING LOTS AND DRIVEWAYS;
C. ROADS;
D. SIDEWALKS;
E. ALL JURISDICTIONAL WETLAND AREAS AND/OR WATERS OF THE
U.S./STATE; AND
F. FLOODPLAINS.
THE FILE MUST BE APPROVED BY ENVIRONMENTAL ENGINEERING
PRIOR TO ISSUANCE OF THE LAND DISTURBANCE PERMIT.
61. A LAND DISTURBANCE PERMIT IS REQUIRED FOR THIS PROJECT
AND SUBSTANTIAL OR FULL SITE PLAN APPROVAL IS REQUIRED
PRIOR TO ITS ISSUANCE. A LAND DISTURBANCE PERMIT IS
A PREREQUISITE TO A BUILDING PERMIT.
62. A LAND DISTURBANCE PERMIT IS REQUIRED FOR THIS PROJECT
AND THE FOLLOWING ARE REQUIRED PRIOR TO ITS ISSUANCE:
A. SUBSTANTIAL OR FULL SITE PLAN APPROVAL
B. THE VIRGINIA STORMWATER MANAGEMENT PERMIT (VSMP)
REGISTRATION STATEMENT AND FEE OF $2700
C. THE STORMWATER POLLUTION PREVENTION PLAN (SWPPP) BOOK
D. POSTING OF AN EROSION AND SEDIMENT CONTROL SURETY
