04. A BMP MAINTENANCE FEE IN THE AMOUNT OF $1,500 PER
IMPERVIOUS ACRE IS REQUIRED FOR COMMERCIAL DEVELOPMENT
PAYMENT MUST BE RECEIVED PRIOR TO ISSUANCE OF A LAND

05. ADDITIONAL EROSION CONTROL MEASURES OTHER THAN THE
MINIMUM STANDARDS CONTAINED IN THE VIRGINIA EROSION AN
SEDIMENT CONTROL HANDBOOK WILL BE REQUIRED IF IT IS
DETERMINED THAT SUCH MEASURES ARE NECESSARY FOR
PROTECTION OF SENSITIVE ENVIRONMENTAL FEATURES AND/OR

06. IT IS THE RESPONSIBILITY OF THE OWNER TO PROVIDE ONE
ADDITIONAL SET OF PLANS TO THE ENVIRONMENTAL ENGINEERI

07. PRIOR TO ISSUANCE OF A LAND DISTURBANCE PERMIT, THE
NAME, "WALK-IN" ADDRESS, AND TELEPHONE NUMBER OF
A LOCAL REGISTERED AGENT REPRESENTING THE OWNER FOR
SERVICE OF PROCESS MUST BE PROVIDED ON THE "APPOINTMEN
OF AGENT FOR SERVICE OF PROCESS" FORM FOUND ON THE
CHESTERFIELD COUNTY ENVIRONMENTAL ENGINEERING WEBPAGE.

08. PRIOR TO ISSUANCE OF A LAND DISTURBANCE PERMIT, A COPY
OF THE AUTOCAD .DWG OR .DXF MUST BE SUBMITTED TO
CAROLE CLIBORNE OF ENVIRONMENTAL ENGINEERING. THE
AUTOCAD FILE MAY BE SUBMITTED ON A CD, DVD, OR VIA
EMAIL. THE FILE MUST CONTAIN THE FOLLOWING, EACH
CLEARLY IN SEPARATE LAYERS:
A. PROPOSED BUILDING FOOTPRINT;
B. ALL IMPERVIOUS AREAS (PARKING LOTS, DRIVEWAYS, ROADS,
ETC);
C. THE STORM SEWER SYSTEM AND ALL CULVERTS;
D. ALL JURISDICTIONAL WETLAND AREAS AND/OR WATERS OF THE
U.S./STATE;
E. STREAMS AND OTHER WATER BODIES;
F. LOCATIONS OF FIELD VERIFIED RESOURCE PROTECTION AREA
DESIGNATIONS TO INCLUDE WATER BODIES WITH PERENNIAL
FLOW, TIDAL SHORES, TIDAL WETLANDS, NON-TIDAL WETLANDS
WHICH ARE CONNECTED AND CONTIGUOUS TO TIDAL WETLANDS O
PERENNIAL FLOW;
G. LOCATIONS OF PERENNIAL FLOW ORIGINS AND THEIR ASSOCIAT
GPS COORDINATES (NAD83 STATE PLANE VA SOUTH FEET);
H. LOCATION(S) OF BREAK POINT(S) IN SURFACE FLOW AND/OR
NON-CONTIGUOUS WETLANDS AND THEIR ASSOCIATED GPS
COORDINATES; AND
I. FLOOD PLAINS.
ALL AUTOCAD FILES MUST BE SAVED AS A .DWG OR .DXF IN
AUTOCAD VERSION 2009 OR EARLIER AND REFERENCED DIRECTL
TO THE VIRGINIA STATE PLANE COORDINATE SYSTEM, SOUTH
ZONE IN THE NAD83 DATUM. THE CD, DVD, OR EMAIL MUST B
LABELED WITH THE SITE PLAN NAME, SITE PLAN NUMBER,
ENGINEERING FIRM AND CONTACT NAME AND PHONE NUMBER.
09. A LAND DISTURBANCE PERMIT IS REQUIRED FOR THIS PROJECT
AND SUBSTANTIAL OR FULL SITE PLAN APPROVAL IS REQUIRED
PRIOR TO ITS ISSUANCE. A LAND DISTURBANCE PERMIT IS
A PREREQUISITE TO A BUILDING PERMIT.
10. A LAND DISTURBANCE PERMIT IS REQUIRED FOR THIS PROJECT
AND THE FOLLOWING ARE REQUIRED PRIOR TO ITS ISSUANCE:
A. SUBSTANTIAL OR FULL SITE PLAN APPROVAL
B. A VDOT LAND USE PERMIT
C. COPIES OF THE VIRGINIA STORMWATER MANAGEMENT PERMIT
