Jury Questionnaire

What is the jury questionnaire, why did I receive it and what do I do with it?

  • mailing annually approximately 14,000 juror questionnaires to randomly selected Chesterfield County residents
  • reviewing responses by Jury Commissioners appointed by the Chief Judge and the processing of the Commissioners’ decisions
  • creating a list of citizens qualified to serve petit (trial) jury duty
  • completing the questionnaire within the time period specified on the form is required
    • complete online or you may complete paper form, sign and return it in the postage-paid envelope provided
    • follow the instructions provided on the questionnaire
    • provide the specific information requested, it is required for several possible exemption

Does receipt of this questionnaire mean I have to serve jury duty?

Receipt of a jury questionnaire does not mean that you are required to serve jury duty.

  • Completing the questionnaire within the time period specified on the form is required.
    • complete online or you may complete it by hand, sign it and return it in the postage-paid envelope provided
    • follow the instructions provided on the questionnaire

Questionnaires Believed to Have Been Received in Error

I no longer live in Chesterfield County. The questionnaire was forwarded to my new address by the post office or a family member. What do I need to do?

You may complete and return the paper questionnaire or you may complete the questionnaire online.

Paper questionnaire 

  • provide your non-Chesterfield address on the form in the GENERAL INFORMATION section
  • answer the questions dealing with being a resident of Chesterfield and Virginia in the QUALIFICATIONS section of the questionnaire

NOTE: Address changes outside of Chesterfield County cannot be processed using the online address update, enter your new address in the text box located near the bottom of the Juror Information page

Individuals who are no longer residents of Chesterfield County can have their name removed from the Voter Registration List by

A jury questionnaire was delivered to my address but it is addressed to someone who does not live at my address. What should I do?

  • If the questionnaire has not been opened
    • please write on the face of the envelope "Return to Sender: Not at This Address"
    • put it back in the mail
  • If the questionnaire has been opened
    • write next to the printed name and address "Addressee Not At This Address,"
    • place it in the prepaid return envelope provided
    • put it back in the mail

A jury questionnaire has been received for a deceased family member. What do I do?

Write 'Deceased' on the questionnaire next to the preprinted name and address and return the questionnaire in the postage-paid envelope provided.

Requests for Exemption

The jury questionnaire is addressed to my son/daughter who is a student temporarily living outside of Chesterfield County. What do I need to do with the questionnaire?

  • All whom receive a jury questionnaire are required to complete it within the time specified on the form.
  • The questionnaire may be completed online.
  • Or complete the paper form questionnaire within the time period specified on the form is required by hand
    • follow the instructions provided on the questionnaire
    • sign and return it in the postage-paid envelope provided
  • There is no statutory exemption from serving jury duty for students.
  • When a student is selected to serve jury duty, the jury coordinator must work with the student to find a time when the student can be available to serve.

If I am currently making vacation plans or have already made vacation plans for next year, will I be required to cancel my vacation if I am selected to serve jury duty?

  • The Clerk’s Office cannot excuse anyone from completing the jury questionnaire.
  • Completing the questionnaire within the time period specified on the form is required.
    • complete online or you may complete paper form, sign and return it in the postage-paid envelope provided
    • follow the instructions provided on the questionnaire
  • If you are selected to serve as a juror, inquiries are made regarding vacations and business travel already planned at that time and consideration will be given to that.

I have an illness or disability that I believe would prevent me from serving jury duty. There is no place on the jury questionnaire form to indicate this. How/where/when do I do communicate this to you?

  • You are correct that there is no exemption in the POSSIBLE EXEMPTIONS section on the jury questionnaire for personal illness or disability.
  • Completing the questionnaire within the time period specified on the form is required.
    • complete online or you may complete paper form, sign and return it in the postage-paid envelope provided
    • follow the instructions provided on the questionnaire
  • If you are selected to serve as a juror, you will have an opportunity at that time to communicate any illness or disability that you believe prevents you from serving jury duty.

Web Submissions and Troubleshooting

I tried to complete the questionnaire online but the internet address given on the questionnaire does not display the appropriate screen. What can I do?

  • The application is compatible with the following browsers/versions
    • Internet Explorer Version 7 and higher
    • Firefox Version 10 and higher
    • Chrome Version 10 and higher
    • Safari Version 4 and higher
  • The web address must be entered exactly as shown on the questionnaire.
  • The web address needs to be entered in the address field of the browser, not in a search field.
  • Entering www instead of https may not work.

If you are still unable to access the login screen for the web entry software, you will need to

  • complete the paper form questionnaire within the time period specified on the form
  • follow the instructions provided on the questionnaire
  • sign and return it in the postage-paid envelope provided

I am trying to complete the questionnaire online but it will not accept my login values. What am I doing wrong?

When the system does not recognize correctly entered login values, there is an inconsistency in the values you are entering for the birth date and zip code with those stored in the system.

  • The birth date stored in the voter registration data (and perhaps shown on your voter registration card) may be incorrect.
  • Check the birth date shown on your voter registration card and try entering that.
  • Please consult the instructions on the login screen and enter the birth date exactly as instructed.
  • The 5-digit zip code printed on the questionnaire is the zip code that must be initially entered, even if it is incorrect or is the zip code for a previous address.

If you are still unable to successfully log in to the web entry software, you will need to

  • complete the paper form questionnaire within the time period specified on the form
  • follow the instructions provided on the questionnaire
  • sign and return it in the postage-paid envelope provided

I am trying to complete the questionnaire online but it will not accept the entry of my new address. What am I doing wrong?

The online address update feature of the system will accept the entry of a change of address for addresses located in Chesterfield County only.

NOTE: You cannot enter a change of address for an address outside of the county.

You may still complete the questionnaire online

  • enter your new address in the text box located near the bottom of the Juror Information page

Or

  • complete the paper form questionnaire within the time period specified on the form
  • follow the instructions provided on the questionnaire
  • noting your new address outside of Chesterfield County
  • sign and return it in the postage-paid envelope provided

I know I received a jury questionnaire but I have misplaced the form. Where can I get the juror id number assigned to me so that I can complete the questionnaire online?

The Clerk’s Office has access to the 9-digit juror ID number that was printed on the original jury questionnaire you received.

  • call the Clerk’s Office, ask to speak with the Jury Coordinator to get your 9-digit juror ID number

Name Changes and/or Corrections

The name printed on my jury questionnaire is incorrect or I have changed my name. What do I need to do?

Provide the correct spelling or change of your name next to the pre-printed name and the GENERAL INFORMATION section of the questionnaire.

Addressee Out of Area

The jury questionnaire is addressed to a family member who is currently out of the area or out of the country for an extended period of time and will not be available to complete the questionnaire in the time specified. What should I do?

If the family member has access to a computer and the internet, they can attempt to complete the questionnaire online. You will need to provide them with the

  • web address
  • 9-digit juror id number that is printed on the bottom of the jury questionnaire
  • zip code from the address to which the questionnaire was sent

If your family member is not able to complete the questionnaire online, you may complete and sign it on their behalf with their permission, noting the reason why they are not able to complete it.

If you do not wish to complete and sign the questionnaire on your family member’s behalf, please note on the questionnaire why the person is not able or available to complete the questionnaire and return it in the postage-paid envelope provided.
