There are different forms required by the County for various types of permits.  These forms are downloadable in PDF format for your convenience. Forms for all Houses

• Building Permit Application - to request a building permit,
• Residential Plans Review Delivery Form - to accompany any building permit application
• Mechanical/Boiler Permit Application - for all mechanical work,
• Electrical Permit Application - for all electrical work,
• Plumbing/Gas Permit Application - for all plumbing and gas related work,
• Credit Card Authorization Form - Discover, Master Card and Visa
• Certified Responsible Land Disturber (CRLD) -  required of the responsible person who will be in charge of the erosion, sediment and dust control plan.

Forms for Special Situations

• Swimming Pool Affidavit - required from a homeowner who is getting a barrier and electrical permit for a swimming pool. See the informational sheet for swimming pools.
• Residential Amendment - required when plans change and a re-approval by the Planning or Health Department is necessitated,
• Multi-Unit Application - for improvement work on townhouses and condos,
• Residential Master Plan Application - if the same construction documents are going to be reused for multiple different sites,
• Refund Request - form to request a refund,
• Fire Permit Application - for any work dealing with a fire sprinkler system,
• Asbestos and Demolition Certification Form - for use when a structure is going to be razed (and a new structure is not going to replace it)
