Service Coordination

Service Coordination staff assist individuals with mental retardation in learning about and accessing resources, supports and services within their community.

Another component of service coordination is the Family Intervention Program.  It provides flexible and individualized behavioral supports to individuals and their families in their own home.  The goal is to assist families with individuals who have mental retardation.

Family Intervention Supports

• intensive in-home supports
• specialized family supports
• behavioral consultations
• family workshops
• disability awareness training

Supports Include:

• intake screening
• ongoing service coordination
• individual service planning and monitoring
• information and referral
• advocacy
• school transition planning
