First Appointment Requirements

What do I need to bring with me for my first appointment?

We ask that you arrive approximately 30 minutes before your appointment time.  You will need the following information in order to complete the paperwork process:

  1. Verification of the gross income for your family (this includes your income and your spouse’s income if you are married).  This can be in the form of a current pay stub(s), W-2 forms, most recent income tax return, etc.  Please include verification of any Social Security, Social Security Disability, Supplemental Income or child support payment information you receive for yourself or a family member.
  2. Insurance card(s).  This includes Medicaid and Medicare.
  3. Name, address and telephone number of two people (family or friends) that can be contacted in case of an emergency.
  4. Name, address and telephone number of your primary physician, if you have one.
