Adult, Family and Children's Services Programs
Adult Services/Adult Protective Services
The Family Assessment and Adult Services team provides a variety
of supportive services designed to empower our customers to achieve
their full potential.
Adult Services
Adult Services covers programs for companion services and nursing
home pre-screenings.
Companion Services is a program available to income eligible elderly
and disabled adults who need help with basic daily living activities
such as light housekeeping, meal preparation, shopping, medication
monitoring, etc.
Nursing home pre-screening assessments are provided for those citizens
that are age sixty-five or older, disabled adults or children that
are in need of community based care (in-home nursing home care)
or require twenty-four hour care in a nursing home facility. A social
worker and a health department nurse complete the pre-screening
assessment. The assessment determines the individual's medical need
for care. Pre-Admission Screenings are provided at no cost.
To schedule a Pre-Admission Screening, you may call the Chesterfield
- Colonial Heights Department of Social Services, (804) 748-1100,
between the hours of 8:30 a.m. and 5:00 p.m. weekdays and ask for
Service Intake.
Medicaid is a resource to assist with
nursing home and community based care costs.

finances uses the money for his or her own purposes without the
consent of the adult
- Sexual abuse, which includes any form of sexual contact (intercourse,
exposing body parts, touching, etc.) without consent
If you suspect that an elderly or disabled adult is being abused,
